Insurance producers may charge fees in addition to receiving commissions for insurance placement.  Wis. Stat. Ann. § 628.32. 

Yes.  Before charging a fee to an insured, an insurance producer must provide written disclosure of the amount of the fee to be paid by the insured and, to the extent applicable, the amount of any compensation from other sources (such as commissions).  Wis. Stat. Ann. § 628.32.

Cap found unconstitutional in Mayo v. Wisconsin, 2017. However, cap was reinstated in 2018 by Wisconsin Supreme Court. Non-economic: $250,000, adjusted for inflation annually with an absolute maximum of $375,000. In catastrophic cases, $500,000 adjusted annually up to a max of $750,000. No cap on economic damages
